Can AI Replace Therapy? Exploring the Promise—and the Real Risks

As conversations about mental health become more open and accessible, it’s not surprising that many people are turning to AI tools for support. They’re instantly available, non‑judgemental, and free or low-cost. For anyone facing long waiting lists, limited services, or the vulnerability of reaching out to a stranger, these tools can feel like a lifeline.

There are meaningful advantages to using AI in wellbeing. But as an integrative therapist and someone who works deeply with identity, culture, neurodiversity, relationships and sexuality, I also hold a clear boundary: AI can support emotional wellbeing, but it cannot safely replace psychotherapy.


Let's explore both sides: what AI can genuinely offer, and where the limits become not just important, but potentially dangerous.


The Advantages: Where AI Can Be Helpful


1. Immediacy and Convenience

AI is always available. For someone who needs a grounding technique at 2am, or a quick reflection before a difficult conversation, that can genuinely help.


2. A Low-Pressure First Step

Speaking to a human therapist can feel exposing, especially around cultural identity, sexuality, trauma, or neurodiversity. AI can offer a low-stakes space to put feelings into words and start noticing patterns.


3. Support Between Sessions

For clients already in therapy, AI can help them practice reflective writing, rehearse difficult conversations, or draft CBT-style worksheets—useful, but not therapeutic in itself.


4. No Fear of Judgment

AI doesn’t react with shock or discomfort. That can make it easier for some people to articulate thoughts they feel ashamed of. But as we’ll see, this can quickly become unsafe.


The Limitations: Where AI Cannot Replace Therapy


There are clear, research‑supported concerns about AI stepping into the role of a therapist. These risks aren’t abstract - they’re practical, clinical and ethical. And they matter most for the very people who need the most support.


1. AI Produces “Confident Nonsense”

AI can sound warm, wise and plausible… while being clinically incorrect. In mental health, slightly wrong advice can reinforce avoidance, validate unsafe behaviour, or deepen shame.


2. No Assessment, No Diagnosis, No Formulation

Therapy isn’t just “talking kindly”. It begins with understanding what is actually going on:

  • Is this trauma?

  • Grief? Depression?

  • Bipolar disorder? OCD? Psychosis?

  • A neurodivergent profile?

  • A physical health condition showing up emotionally?


    AI cannot distinguish between these—and cannot gather the nuanced clinical information required to make sense of symptoms.



3. AI Cannot Manage Risk or Safeguarding

This is the most serious limitation. In moments of crisis—suicidality, self-harm, domestic abuse, coercive control—AI cannot:

  • assess real-world risk

  • recognise cues in tone or behaviour

  • coordinate with crisis teams, GPs, or safeguarding bodies

  • initiate emergency pathways

AI may continue the conversation, appearing to be comforting, while offering no actual safety.


4. Risk of Reinforcing Delusions or Disordered Thinking

LLMs are trained to be agreeable. That means they may mirror a user’s beliefs—even if those beliefs are distorted or dangerous. This can worsen:

  • distortions in perception

  • obsessive spirals

  • negative self-beliefs

  • paranoid or intrusive thinking


5. Dependency and Social Withdrawal

Because AI is endlessly available and endlessly validating, some people start relying on it instead of real relationships. This can deepen isolation—a factor that maintains many mental health conditions.


6. Privacy and Confidentiality Risks

Unlike therapy, where confidentiality is governed by strict professional and legal frameworks, AI tools often store, analyse or repurpose data. Your most intimate disclosures can become part of an opaque system and there are serious GDPR and UK data governance concerns in this area.


7. Bias and Cultural Incompetence

AI can unintentionally reinforce stereotypes or misunderstand key cultural, sexual, or neurodivergent contexts—areas where many clients already experience marginalisation.


8. No Accountability

If a therapist harms you, there are professional bodies, ethical frameworks and complaints pathways. If a chatbot harms you… who is responsible?


So What’s the Bottom Line?

AI can be a supportive tool, especially:

  • for reflective writing

  • for grounding exercises

  • for practising skills between sessions

  • for organising thoughts

  • for psychoeducation

But it cannot hold the heart of psychotherapy:

  • deep relational attunement

  • skilled assessment

  • ethical responsibility

  • evidence-based treatment

  • safeguarding and crisis care

  • a safe, boundaried human relationship that can tolerate complexity and emotion


As a therapist, I see every day how healing happens not through perfect advice, but through connection, curiosity, and the courage to explore one’s inner world in the presence of another caring person.
